Required Forms For Employment Application Medical release Employment Agreement Background check form	The Lavallette Beach Patrol Lifeguards & Badge Checkers Lifeguards Dear Candidate: The Lavallette Beach Patrol announces that the annual test for the position of Lifeguard will be held on Saturday May 17, 2008 at 9:00am . The test will be given at the Ocean County College pool, (Campus Map), which is located on Hooper Ave, in Toms River, New Jersey. Please report to the Outdoor Track, as we will complete the run test first. The Requirements for those wishing to be employed are: 1. Copy of Birth certificate or Driver’s license (16 years of age or older) 2. Copy of Valid CPR certificate (BLS-C) or willingness to complete by starting date. 3. Copy of Valid First Aid certificate - Community First Aid or willingness to complete by starting date. 4. Medical release completed by a physician indicating your physical ability to perform the duties of an ocean lifeguard. 5. Completed Application. 6. Employment Agreement. 7. Pass a Background check (download the form) 8. Working Papers for those under age 18 as of June 21, 2008 . Candidates will be judged on their overall physical fitness and knowledge of the ocean. The Test will consist of three parts: 1. 500 yard swim (must be completed in less then 10 minutes) 2. 300 yard shuttle run 3. Interview The Beach will open for the season on June 21, 2008 . Each guard will work 5 days per week (40 hrs). Work starts each day at 8:45am and ends at 5:15pm on weekdays; till 6:00 on weekends.
